NeuroPace Faces IGE Approval Delay After FDA Regulatory Hurdle

NeuroPace is managing a regulatory delay for its IGE indication following an FDA non-approvable letter, while maintaining its 2024 revenue growth targets.

NeuroPace is recalibrating its regulatory strategy for its idiopathic generalized epilepsy (IGE) indication after receiving a non-approvable letter from the FDA in late July 2024. Despite the setback for this specific expansion, the company maintains its 2024 revenue growth guidance and its path to profitability.

"Of the 3.6 million people in the U.S. with epilepsy, about 1.2 million are drug-resistant, but only about 75,000 reach Level 4 comprehensive epilepsy centers and only about 15,000 receive some form of medical intervention each year," said Patrick Williams, CFO of NeuroPace, during the Wells Fargo Healthcare Conference.

Regulatory Roadblock and Timeline

The FDA’s non-approvable letter cited concerns regarding the representation of pediatric patients in the clinical data and subpopulation analyses concerning seizure frequency. The company’s NAUTILUS study for IGE met its safety endpoint but failed to reach its primary endpoint of time to the second generalized tonic-clonic seizure. Secondary results showed a 100% median seizure reduction at the 24-month mark.

NeuroPace management expects to meet with the FDA in early to mid-Q4 2024 to discuss the Submission Issue Request process. This regulatory path could necessitate a major amendment to the application, which may push final approval into the first or second quarter of 2027.

Financial Outlook and Market Position

While the IGE indication represents a growth opportunity, NeuroPace reaffirmed its 2024 revenue growth guidance of 21% to 23% for its core RNS System. The company also maintains its financial objective of achieving cash flow breakeven by the end of 2027. Management stated this target is independent of the IGE approval timeline.

The FDA’s non-approvable letter is a formal communication indicating that an application is not ready for approval in its current form. In the context of medical devices, this process often triggers a major amendment that resets the agency’s review clock. For manufacturers, this requires addressing specific data gaps, such as pediatric representation, before the agency will reconsider the submission. The company’s focus on Level 4 epilepsy centers highlights the specialized nature of the RNS System, which provides responsive neurostimulation for patients who have not responded to traditional pharmacological treatments.
