Humana shares and options activity surged after a Medicare Advantage star ratings upgrade from the Centers for Medicare & Medicaid Services pushed 95% of its members into plans rated four stars or higher, reviving eligibility for lucrative 2028 bonus payments.

The recovery marks a sharp reversal from the previous year, when only 20% of Humana's membership sat in plans meeting that quality threshold. The regulatory boost immediately caught the attention of options traders and Wall Street analysts following a bruising stretch for the health insurer's managed-care margins.

"The improved ratings reflect the work of thousands of Humana employees to help members access high-quality care and coordinated support to improve their health," said Jim Rechtin, Chief Executive Officer, Humana, in a statement regarding the ratings announcement.

Market Reaction and Analyst Projections

The ratings rebound triggered heavy volume across Humana (NYSE: HUM) options and equity markets. As of October 15, 2026, at 20:00 UTC, Humana shares traded at $432.20, marking a 1.12% increase from their previous close of $427.40, according to Finnhub market data.

Financial institutions quickly updated their financial models to account for the restored incentive revenue. According to an Investing.com report, equity research analysts at Evercore estimate the insurer will collect approximately $4.8 billion in bonus payments in 2028 as a direct result of the higher star designations.

Understanding Medicare Advantage Star Ratings

Managed-care organizations rely heavily on the federal star ratings system, which grades plans on a scale of one to five stars based on clinical outcomes, customer service, and preventative care measures. Plans achieving four stars or higher unlock critical bonus payments from the federal government, which insurers typically deploy to lower member premiums, expand dental and vision benefits, and bolster operating margins.

Dropping below the four-star threshold in prior assessment cycles had compressed Humana's profitability and weighed heavily on investor sentiment across the healthcare sector. The return to a 95% high-rating tier removes a major earnings overhang that had previously forced the company to scale back certain supplemental benefits to remain competitive against rivals like UnitedHealth Group and CVS Health's Aetna.

What the Rebound Means for Managed Care

The regulatory reprieve highlights the high-stakes nature of federal quality metrics for private insurers operating within government healthcare programs. With billions in forward bonus revenue secured through the 2027 assessment, Humana enters upcoming contract bidding cycles with stabilized financial projections, alleviating immediate pressure on its capital allocation strategy.
