Structure Therapeutics detailed clinical progress for its oral obesity and diabetes pipeline at the Morgan Stanley healthcare conference, highlighting trial designs and upcoming milestones for lead candidate aleniglipron. The clinical updates mark a transition from earlier development stages as the company prepares to enroll thousands of participants in late-stage trials.
"We've shown 16 percent weight loss at the 180 milligram dose after 7 to 8 weeks on the top dose, with a 2.6 percent discontinuation rate due to adverse events in Phase II," said Ray Stevens, Chief Executive, Structure Therapeutics, during the presentation.
Advancing Phase III Trials
Structure Therapeutics is advancing two Phase III studies for aleniglipron, designated as ACCOMPLISH-1 and ACCOMPLISH-2. According to the company presentation, these trials will enroll about 5,000 participants in total. Data from the Phase III program is expected at the end of 2028.
The large-scale trials follow mid-stage safety and efficacy data that demonstrated tolerability alongside double-digit weight reduction. In clinical trial design, Phase III studies expand participant numbers significantly to confirm therapeutic benefits and monitor rare adverse events prior to regulatory submission.
Oral Amylin Program and Technical Hurdles
Beyond aleniglipron, the company highlighted its newer oral amylin program, ACCG-2671, which is currently in Phase IIa development. Amylin is a hormone co-secreted with insulin that regulates gastric emptying and promotes satiety.
"Oral small-molecule development for the amylin receptor had previously been considered infeasible," said Ray Stevens, Chief Executive, Structure Therapeutics, describing the compound as a technical breakthrough for oral therapeutics.
Developing oral small-molecule agonists for peptide receptors historically presents chemical hurdles because enzymes in the gastrointestinal tract typically degrade peptide structures before absorption. Small-molecule alternatives aim to mimic peptide hormone functions while remaining stable in pill form.
Upcoming Fourth-Quarter Milestones
Structure Therapeutics expects several Phase II updates in the fourth quarter. These upcoming data disclosures include:
- A 44-week body composition study evaluating long-term fat versus lean mass changes.
- A type 2 diabetes study expanding dosing regimens to 180 milligrams.
- A switch study testing patient movement from injectable GLP-1 receptor agonists to oral aleniglipron.
These studies address clinical questions regarding sustained weight loss quality and therapeutic interchangeability between injectable biologics and emerging oral alternatives in metabolic disease treatment.
