Royalty Pharma Eyes Modest 1.1x Return Despite Pelacarsen Setback

Royalty Pharma expects to recoup its investment and secure a modest positive return from Ionis Pharmaceuticals, even after Novartis reported a late-stage clinical trial failure for the cardiovascular drug pelacarsen. The financial cushion stems from a structured, multi-asset funding agreement that balances risk across different pharmaceutical products.
Under the January 2023 funding agreement between the two firms, Royalty Pharma deployed a total of $500 million. That capital was split into $150 million allocated to royalties on pelacarsen and $350 million directed toward royalties on Spinraza, an established treatment for spinal muscular atrophy.
How Structured Funding Shields Investors
Clinical trial failures in the biotechnology sector routinely trigger sharp sell-offs and total write-downs for single-asset investors. However, multi-product funding agreements are intentionally engineered to isolate downside risk by bundling high-risk pipeline candidates with commercial-stage assets that generate predictable, ongoing cash flows.
When Novartis announced that the Phase 3 HORIZON trial failed to meet its primary endpoint of reducing cardiovascular events in patients with atherosclerotic cardiovascular disease and elevated lipoprotein(a), the pelacarsen royalty stream was heavily compromised. Despite that clinical setback, the structural terms of the Ionis deal protect Royalty Pharma's principal through its exposure to Spinraza.
Mechanics of the Spinraza Reversion
Following the unsuccessful HORIZON trial results, the contractual terms governing the funding arrangement shift. Royalty Pharma's royalty interest in Spinraza will revert to Ionis once cumulative royalty payments hit $550 million.
This threshold guarantees a 1.1x return on the total $500 million originally deployed across the portfolio. Market reaction to the news remained muted; Royalty Pharma PLC (RPRX) closed at $63.96, up 0.13% from its previous close of $63.88, as of September 4, 2026, according to Finnhub market data.
What the HORIZON Trial Failure Means
The outcome of the HORIZON trial marks a hurdle for pelacarsen, an antisense therapy designed to lower lipoprotein(a) levels, which is a known independent risk factor for cardiovascular disease. Novartis has not yet detailed the full future development path for the therapy following the missed primary endpoint.
For specialty finance firms like Royalty Pharma, the episode highlights the utility of portfolio diversification within single corporate partnerships. By tying early-stage or high-risk clinical candidates to approved blockbusters, funders insulate their balance sheets from binary clinical trial outcomes while still retaining upside exposure if experimental drugs succeed.
