AstraZeneca Phase III Etcamah Trial Misses Survival Endpoint
AstraZeneca announced its Phase III SERENA-4 trial of Etcamah with palbociclib missed its primary progression-free survival endpoint.

AstraZeneca PLC announced that its Phase III SERENA-4 trial evaluating the oral selective estrogen receptor degrader Etcamah, also known as camizestrant, in combination with palbociclib did not meet its primary progression-free survival endpoint for first-line advanced breast cancer treatment. The setback halts the current combination strategy in untreated advanced stages of the disease, though the pharmaceutical company is maintaining its broader clinical evaluation pipeline.
"Whilst we are disappointed by the SERENA-4 outcome, it sharpens our focus on maximising the number of patients who can benefit from Etcamah today based on SERENA-6," said Susan Galbraith, Executive Vice President, Oncology Haematology R&D at AstraZeneca, in a statement regarding the trial results.
Trial Design and Patient Population
The global SERENA-4 study enrolled 1,371 adult patients who were newly diagnosed with Stage IV de novo or recurrent breast cancer. Participants in the trial had not previously received any systemic treatment for their metastatic disease, establishing a baseline of treatment-naive patients for the combination therapy of Etcamah alongside palbociclib.
Palbociclib is a standard cyclin-dependent kinase 4 and 6 (CDK4/6) inhibitor commonly used in combination with endocrine therapies to treat hormone receptor-positive, human epidermal growth factor receptor 2-negative breast cancers. The trial aimed to determine whether adding camizestrant to this standard backbone would significantly delay disease progression compared to existing first-line options.
Pipeline Context and Upcoming Evaluations
Despite the primary endpoint miss in the SERENA-4 trial, AstraZeneca continues to investigate camizestrant across other clinical settings. The company's ongoing research includes the SERENA-6 trial, which evaluates the therapy in earlier lines of intervention by detecting detectable mutations before clinical progression occurs.
Additionally, the broader evaluation of camizestrant involves the CAMBRIA clinical trial program, which looks at the molecule in adjuvant settings for patients with early-stage breast cancer. These concurrent evaluations represent alternative pathways for the compound as researchers analyze the complete dataset from the failed combination study.
